Your account number (found on your billing statements in the Tilt app)
The name of the credit reporting agency with the incorrect information
The specific item(s) you believe are inaccurate — include the date, description, and any details that help us locate them
What you believe is incorrect or incomplete, and why
If you already filed a dispute directly with any credit bureau?
A copy of your credit report with the disputed item(s) highlighted (optional, but helpful)
